What is a Chronic Condition Management Plan (CCMP)?

A CCMP is a personalised plan that you create with your GP. It’s designed for people with chronic or terminal conditions that have lasted, or are likely to last, for six months or more.

This plan replaced the older “Chronic Disease Management Plan (CDMP)” and “Team Care Arrangement (TCA)” in 2025.

The main benefit of this plan is that it unlocks up to five (5) Medicare-subsidised appointments per calendar year with allied health providers, such as your podiatrist.

Are You Eligible for a Podiatry Rebate?

Only your GP can determine if you are eligible and create a CCMP. You may qualify if you have a chronic condition that is being managed by your doctor.

Common conditions we treat under a CCMP include:

  • Diabetes: For high-risk foot assessments and neurovascular checks.
  • Arthritis: (Osteoarthritis or Rheumatoid) For managing joint pain, stiffness, and mobility.
  • Chronic Foot Pain: For conditions like long-term plantar fasciitis, Achilles pain, or tendon issues.
  • Other Conditions: Any other chronic issue that affects your foot health and mobility.

Your Simple 3-Step Process

Accessing your rebate is a straightforward process managed by your GP.

1. Visit Your GP

Book an appointment with your doctor to discuss your chronic condition and ask if you are eligible for a CCMP.

2. Receive Your Referral

If you are eligible, your GP will create the plan and provide you with a referral form for podiatry services.

3. Book Your Appointment With Move Podiatry

Call our Manly clinic or book online. Please let our team know you have a CCMP referral when you book, and remember to bring your referral paperwork and Medicare card to your appointment.

Frequently Asked CCMP Questions

How many appointments can I get?

The CCMP provides a total of five (5) subsidised sessions per calendar year. These 5 sessions are shared across all allied health services your GP refers you to (e.g., podiatry, physiotherapy, dietetics). Your GP will decide how these are allocated.

How much is the Medicare rebate?

A CCMP referral provides a significant Medicare rebate for your podiatry sessions. As rebate amounts are updated annually by Medicare, please ask your GP or our reception team for the current rebate amount at the time of your booking.

Do I have to pay the full fee upfront?

Yes. We process the full consultation fee at the time of your appointment. We can then process your Medicare rebate for you on the spot, and it will be paid directly into your nominated bank account. You are only “out-of-pocket” the remaining gap fee.

What if I have an old CDMP or TCA referral?

If you received a referral under the old Chronic Disease Management Plan (CDMP) or Team Care Arrangement (TCA) before July 2025, it remains valid until June 30, 2027. We still accept these valid referrals.
